This Disability Motiv Brands, Affiliates, and Marketing Partners Disclosure explains who operates the Disability Motiv lead-generation experience, how Disability Motiv lead requests may be processed, and who may contact consumers after they submit a request for disability benefits assistance and related services.
This disclosure supplements the Disability Motiv website experience, the Privacy Policy, the Terms of Use, and any consent language presented at the time a consumer submits a form, call request, eligibility check, survey, or other inquiry.
1. Disability Motiv Brand Ownership and Operation
Disability Motiv is a consumer-facing marketing brand and DBA of Leads Motiv, LLC, a subsidiary of Motiv Brands, Inc.
Disability Motiv is used for marketing, consumer education, lead generation, eligibility reviews, consultations, provider matching, and related commercial activities involving disability benefits assistance and related services.
Use of the name Disability Motiv does not necessarily mean that Disability Motiv is a separate legal entity.
Depending on the applicable website, form, campaign, service, or business arrangement, the Disability Motiv website or lead-generation experience may be operated, supported, processed, or administered by Leads Motiv, LLC; Motiv Brands, Inc.; another Motiv-affiliated company; vendors, technology providers, call centers, or service providers acting on behalf of one of the foregoing; or selected third-party advocates, representatives, or law firms.
2. Purpose of the Disability Motiv Lead Request
When a consumer submits information through Disability Motiv, the information may be used to help evaluate, process, route, match, sell, license, transfer, or otherwise provide the consumer’s request to one or more companies that may be able to assist with the requested service.
A Disability Motiv lead request may be used to review the consumer’s inquiry; determine the consumer’s general location and state; identify potentially available representatives; match the consumer with an advocate, representative, or law firm; route the request through lead-distribution, matching, ping/post, bidding, or similar systems; connect the consumer with a selected participant; facilitate calls, text messages, emails, appointment scheduling, or follow-up communications; maintain records of the consumer’s request and consent; perform compliance, fraud-prevention, analytics, auditing, and business operations; and support the services requested by the consumer.
3. Companies That May Contact Consumers
After a consumer submits a request through Disability Motiv, the consumer may be contacted by the participating advocate, representative, or law firm selected for the request; Disability Motiv; Leads Motiv, LLC; Motiv Brands, Inc.; or service providers, vendors, technology providers, or call centers acting on behalf of any of them.

Each participating advocate, representative, and law firm is an independent third party and is not owned by, controlled by, or legally affiliated with Disability Motiv, Leads Motiv, LLC, or Motiv Brands, Inc., unless otherwise expressly stated.
A participating company may contact the consumer regarding the consumer’s disability inquiry using the contact information and communication permissions provided through the Disability Motiv form.
4. Marketing Partners Disclosure
The companies that may receive, purchase, license, process, or contact consumers regarding Disability Motiv leads are identified in the applicable Marketing Partners Disclosure, available at motivbrands.co/disclosures/marketing-partners.
The Marketing Partners Disclosure may include the legal name of the company; the company’s consumer-facing brand or DBA; the products or services the company may provide; the states or geographic areas in which the company may operate; the company’s role in receiving, processing, or contacting leads; and other information reasonably necessary to identify companies that may receive or contact consumers.
A company listed in the Marketing Partners Disclosure may not receive every lead. Lead distribution depends on factors such as the consumer’s requested service, state, ZIP code, service area, participant availability, campaign rules, routing logic, licensing, partner capacity, consumer consent, and applicable law.
Being listed in the Marketing Partners Disclosure does not guarantee that a company will receive a particular consumer’s information or contact a particular consumer.
5. Lead Distribution and Provider Matching
When a consumer submits a request through Disability Motiv, the request may be processed by Leads Motiv, LLC, Motiv Brands, Inc., another Motiv affiliate, or approved service providers for the purpose of matching the consumer with an appropriate participant.
A completed lead may be sold, licensed, transferred, routed, or otherwise provided to a selected participant, lead buyer, marketing partner, or service provider.
Unless otherwise disclosed in the applicable consent language or Marketing Partners Disclosure, a Disability Motiv lead is intended to relate to disability benefits assistance or closely related services.
Depending on the applicable campaign, consumer request, consent language, routing process, or participant arrangement, a lead may be made available to one or more companies where permitted by law and properly disclosed.
6. Communications Consumers May Receive
Consumers may receive communications regarding their Disability Motiv inquiry by telephone call, text message, SMS, MMS, email, or other communication method authorized or permitted by the consumer’s request, consent, and applicable law.
Communications may relate to the consumer’s request; scheduling a consultation; confirming details; reviewing potential eligibility; connecting the consumer with a representative; following up on a submitted request; customer service; compliance or consent verification; or other matters reasonably related to the requested service.
Some communications may be made using automated technology, prerecorded messages, artificial voice, AI-generated voice, automated text messaging, or similar systems where the consumer has provided the required consent and where permitted by applicable law.
Consent to receive marketing calls or text messages is not a condition of purchase. Message and data rates may apply.
7. Lead Recipient Responsibilities
A company that receives a Disability Motiv lead may use the consumer’s information to contact the consumer; evaluate the consumer’s request; determine whether the company serves the consumer’s area; provide or arrange disability benefits assistance; schedule a consultation; perform customer-service or compliance activities; and conduct other activities reasonably related to the service requested by the consumer.
Lead recipients may be subject to their own privacy policies, terms, licenses, regulatory obligations, communication practices, and consumer-choice procedures.
Consumers should review the privacy practices and communications policies of any company that contacts them.
8. No Guarantee of Service, Outcome, or Representative Availability
Submitting a request through Disability Motiv does not guarantee that a consumer will be matched with a representative; that a representative will contact the consumer; that a representative is available in the consumer’s area; that the consumer qualifies for any benefit, program, or service; or that any particular outcome, award, approval, or result will be achieved.
Any services, consultations, agreements, or representation are between the consumer and the applicable participant, unless otherwise expressly stated. Disability Motiv is not a law firm, is not a government agency, and does not provide legal advice or decide any claim.
Disability Motiv is not affiliated with the Social Security Administration or any other government agency, and is not endorsed by any government program.
9. Consumer Communications and Opt-Out Rights
Consumers may request that Disability Motiv, Motiv-affiliated companies, or a lead recipient stop contacting them.
A consumer may submit a request to stop communications by replying STOP to a text message where supported; using an unsubscribe link in an email; contacting the company that sent the communication; clearly stating during a call that the consumer does not want further contact; or contacting Motiv using the information below.
A request to stop communications may apply to a particular company, telephone number, communication method, campaign, or category of communications depending on the request and applicable law.
Motiv may process applicable opt-out or suppression requests and may share suppression information with relevant service providers, vendors, or marketing partners as reasonably necessary to honor the request.
10. Privacy and Consumer Rights
The collection, use, disclosure, sale, licensing, transfer, and retention of personal information through Disability Motiv are governed by the applicable Privacy Policy, this disclosure, the Marketing Partners Disclosure, and any additional notices presented to the consumer.

11. Updates to This Disclosure
This disclosure may be updated from time to time as Disability Motiv operations, ownership structures, lead-routing arrangements, service providers, marketing partners, products, services, or legal requirements change.
